Here’s what you can expect when you show up for your first appointment:
- Check-In & Paperwork:
Quick registration, insurance verification, consent forms. - Consultation & Exam: Review medical history, discuss accident details, and perform a physical exam.
- Diagnostic & Documentation: If needed, imaging or reports to support clinical findings.
- Treatment Plan: Outline recommended therapies (PT, injections, chiropractic, etc.) customized to their condition.
- First Treatment: We start care right away unless diagnostic steps are needed first.
What You Need For Your First Appointment
At T.E.A.M. your recovery journey is completely streamlined to ensure your focus is on getting better. When you first arrive at one of our facilities be sure to bring the following to expedite the process and get right to treatment.
- Insurance card(s) (health, auto, or workers’ comp)
- Claim information if related to an accident (claim number, adjuster contact, case manager info)
- Attorney information, if applicable.
- Medical records or imaging results (X-rays, MRIs, prior treatment notes)
- Medication list (including dosages and frequency)
- Comfortable clothing (athletic wear or loose clothing that allows movement for evaluation/therapy)
